Is the survey maker free?

Yes. It is on the free plan, with 5 forms and 100 responses in total and no card required. The cap is a total rather than a monthly reset, so a survey that runs for a long time at low volume suits it better than one big send.

Can respondents answer anonymously?

Yes, and that is the default. Respondents never create an account and nothing identifies them unless a question on your survey asks. If you need full anonymity, leave out the email question rather than relying on people skipping it.

Can I make a survey without the AI?

Yes. The same builder works by hand: add rating, scale, ranking and multiple choice questions yourself, or start from a survey template and edit it. You can also show the survey one question per screen, which suits longer questionnaires on a phone.

What is the difference between a survey and a questionnaire?

In practice a survey is any set of questions put to a group, while a questionnaire is built so the answers can be analysed together: consistent scales, banded demographics and a deliberate question order. You can build either here; the difference is in how the questions are written rather than in the tool.
