About this template
Club signups fail on collection arrangements more than anything else. A child waiting at the gate because nobody recorded who was allowed to collect them is the failure this form is built to prevent, which is why collection is a required choice with a follow up field for named adults.
Allergies is required rather than optional, with help text telling parents to write none if there are none. A blank allergy field is genuinely ambiguous to a club leader holding a plate of biscuits, and forcing an explicit answer removes the guesswork for the price of one word.
Days and term are separate so you can see capacity per session rather than per child. Add a payment field if clubs are chargeable, and use a rule so the named adults field only appears when collection by a named adult is chosen.
