About this template
Feedback forms fail when they are long and vague. This one asks a question at a time and keeps to what a teacher can act on: an overall star rating, how the pace felt, the most useful part, the one thing to change, and how likely the student is to recommend the class.
The recommend question uses a 0 to 10 scale, so the answers can be tracked over time as a net promoter score and compared between terms or teachers.
Add the teacher's name as a hidden field in the link you share, so responses group by teacher without asking students to type it.

- What questions should a class feedback form ask?
- An overall rating, whether the pace was right, the most useful part, one thing to change, and whether the student would recommend the class. Two open questions are enough; more than that and answers get shorter.
- Can feedback be anonymous?
- Yes. The only personal question is an optional email, so students can leave it blank. Remove it entirely if you want every response to be anonymous.
