About this template
Feedback collected in the last five minutes of the last class is feedback from whoever stayed. This form asks about the teaching, the materials, the pace, and the workload, and it works just as well sent the following week.
Anonymity is offered rather than assumed. Some students want their name against a comment and most do not, and asking outright gets franker answers than either default.
Keep the question set stable between terms. Numbers you can compare are worth more than questions you keep improving.
