About this template
Office hours run badly when six students turn up at once and two of them only needed a sentence. Booking the slots fixes the queue. Each student takes fifteen minutes, says what they are stuck on, and names the module, so the session starts on the question rather than on working out whose coursework this is.
The what have you already tried checkboxes do quiet work. They tell you whether to answer the question or point at the notes, and they nudge students to check the obvious before they book, which is the polite version of the same message.
The default is two afternoons a week, which is what most office hours look like. Change the days, the hours and the slot length to yours. Connect Google Calendar and teaching, meetings and anything else already in your day stop being bookable automatically.
Templates are a starting point, not legal or professional advice. Where a form carries consent, medical, or contractual wording, have somebody qualified review it against the rules that apply to you before you publish.

Frequently asked questions
- How do students book office hours?
- One link for the whole cohort. Students pick a slot, give their course and student ID, and say what they are stuck on, so the time starts on the actual problem.
- Can two students book the same slot?
- No. The appointment field holds one booking per slot, which is what makes a single link workable for a large cohort without a queue outside the door.
- Why ask what they are stuck on?
- Because ten minutes spent looking up the right example beforehand is worth more than ten minutes spent working out what the question is.
