About this template
Most public libraries need the same things before issuing a card: who it is for, their date of birth, a home address in the service area, and proof of both identity and address. This application asks for all of it online, so staff only need to check and print the card.
Card types cover adults, children under 16, seniors, and non residents, and a parent or guardian name is asked for when the card is for a child. Patrons choose which notices they want, due date reminders, holds ready for pickup, or the events newsletter.
Edit the age limits and the documents your library accepts. If non resident cards carry a fee, add a payment field so it is paid when the application is sent.
