About this template
Qualitative research is about experiences and reasons, and long answers come more easily when a participant sees one question at a time. This questionnaire is a sequence of open prompts that moves from context, to a specific recent experience, to feelings, to what they would change.
Consent covers quoting anonymised answers, and participants choose a pseudonym to be quoted under. The last question asks what was missed, which often produces the most useful answer of all.
Change the prompts to fit your topic and keep them open and neutral. The opt in at the end builds a list of people willing to give a follow up interview.
