About this template
Quantitative research needs answers that can be counted. Every question here is closed: age as a number, gender and education as fixed choices, three five point Likert items, hours as a number, a star rating, and a ranking of formats.
Consent is the first question and is required. A hidden participant ID field is filled from a pid value in the link you send each person, so answers can be matched to a sample list without asking for names.
Replace the example statements, about technology and training, with the items from your own instrument, and keep the same scale labels across all of them. Export responses to CSV to load into SPSS, R, or Excel.
