About this template
Most tutoring enquiries arrive as a sentence saying a child needs help with maths, which is not enough to assign anybody. This form asks for the subject and the level, then asks for the goal in the parent's own words, because the gap between wanting a pass and wanting a top grade changes which tutor you send.
Availability is a checklist rather than a free text field for a practical reason: matching a tutor to a student is a scheduling problem before it is a teaching one, and a list of ticked slots can be compared against a tutor's diary in seconds. The current level field gives a baseline so progress can be measured later.
Add a budget field if your rates vary by level, and use conditional logic so choosing online skips the questions about travel. If you run group sessions, add a question about whether the student would share a session.
