About this template
Booking by phone means somebody has to answer the phone. This form takes the booking instead. The appointment field offers real slots from the days and hours you set, one person can hold a slot at a time, and the reason for the visit arrives with it, so nobody has to ring back to agree a time that was never in doubt.
The first visit question matters more than it looks. New clients usually need a longer slot and paperwork the returning ones already have on file, and knowing which you are dealing with before you open the calendar prevents the overrun that follows a fifteen minute assumption.
Set the slot length and opening hours on the appointment field to match how you actually work. Connect Google Calendar and the form will also skip the times you are already booked, then put each new booking on your calendar. Practices that need insurance details up front can add those fields above the contact section.
