About this template
Every booking taken by phone is a booking taken while somebody is mid appointment. This form collects the service, the preferred stylist, the date and time, a backup slot, and the colour history and allergies that change how long the chair is needed for.
Asking what was done recently is the difference between a colour that works and one that has to be corrected. Bleach or straightening in the last few months changes the appointment, and finding out at the basin is the worst time to learn it.
Replace the service and stylist lists with your own, and add a deposit field for the treatments where a missed booking costs you the slot.
