Phone triage at eight in the morning is a queue by another name. This form takes the booking in writing. The patient picks a real fifteen minute slot in surgery hours, describes what they want to be seen about, and says whether one appointment will be enough, which is the question that keeps clinics running to time.
Asking about urgency and length separately is deliberate. A routine review and an ongoing problem getting worse need different responses even when both are booked for next Tuesday, and a patient who says they have three things to discuss can be given a double slot before the clinician discovers it on the day.
The emergency confirmation is there because a booking form is not a triage service and the patient needs to be told so plainly. Connect Google Calendar and the form will not offer a slot during a clinic, a home visit, or annual leave, and each booking appears on the calendar as it is made. Add insurance or referral fields if your practice needs them before the appointment.
