What you need
A Deoochform account and permission to install apps in your Slack workspace.
How to connect it

Open Settings → Integrations in Deoochform and hit Add to Slack.

Pick the workspace and approve the permissions Slack asks for.

Back in Slack, message the app or run /deooch and describe the form you want.

The app replies with the public link. The form is already in your dashboard.
Questions
Can the app read my Slack channels?
No. It only receives the messages you send it directly: a /deooch command, or a DM to the app. It has no access to channel history, and it is never added to a channel to listen.
Who owns a form built from Slack?
You do. The Slack workspace is linked to your Deoochform account, so anything built from it appears in your dashboard and counts against your plan.
Can several people in the workspace use it?
Each person links their own Deoochform account to the workspace, and builds into their own dashboard. One install, separate accounts.
