About this template
Most contact pages offer an email address and hope for the best. This one asks for the four things you need before you can reply: who is writing, how to reach them, what the message is about, and the message itself. The subject field separates sales from support before anyone opens the thread.
Replies get faster when routing happens at submission rather than in a shared inbox. Send partnership enquiries to one address and support to another, or push everything into Slack so whoever is free picks it up first. The phone field stays optional, so a quick question never turns into a form people abandon halfway.
Add a file upload if people need to send screenshots. Remove the phone field entirely if you only ever answer by email.
