About this template
A prospect who asks to be called and then does not answer has usually been called at a time they never agreed to. Letting them pick a day and a window turns a cold dial into an expected one, and connection rates go up sharply for the cost of two extra fields.
The timezone field looks redundant until the first time a team calls somewhere five hours out and leaves a voicemail at six in the morning. Asking for it as free text rather than a long dropdown keeps the form short and the answers are perfectly usable.
Asking what they want to discuss lets whoever calls arrive with the right material, and it routes the enquiry to the right person before the call is even booked. Add a phone extension field if your prospects are on switchboards.
