About this template
Donation pages lose people at the point where the amount is not obvious. This form offers set amounts alongside a free entry, asks whether the gift is one time or recurring, and captures what you need to claim tax relief.
The dedication field is small and matters. Gifts made in memory of somebody arrive with a name attached, and that name belongs in the letter you send back.
Connect a payment processor to take the money at submission. Without one the form records a pledge and you collect separately.

- Yes. Add a donation payment field and the donor chooses the amount, which is what separates a donation from a fixed price product. It is charged through Stripe into your own account.
- Can donors give monthly?
- Yes. The frequency question is on the form, and a subscription payment field charges a recurring gift through Stripe. One off and recurring gifts can sit on the same form.
- Can a gift be made in someone's name?
- Yes. The dedication field is optional and free text, so a donor can name the person the gift honours and you keep it with the record for the acknowledgement.
