About this template

A pledge is worth recording properly because it is a commitment rather than a completed transaction. Amount and frequency are separate fields so a monthly pledge is never mistaken for a single gift, which is the most common error in fundraising records.

The recognition question has three distinct answers, and the middle one matters most. Plenty of donors are happy to be counted publicly but not named, and treating anonymous and no acknowledgement as the same thing loses supporters who wanted to be thanked quietly.

The gift aid declaration is why the address field is required, and the help text explains that rather than leaving people wondering. Asking about employer matching costs one field and occasionally doubles the gift, which is the best return on a question in this form.
