About this template
Sponsorship requests that lead with the amount get declined, because a sponsor is buying reach rather than making a donation. This form puts audience size and audience description before the ask, which is the order a sponsor evaluates in anyway.
The benefits field asks for one per line and specifically for placement and reach, since that is what a marketing budget has to be justified against. A request offering a logo on a website is a different proposition from one offering a speaking slot and a mailing list, and the difference should be visible without a call.
Asking who has sponsored before is the closest thing to a reference in this context. Add a payment field if you sell fixed tiers self serve, and make the decision deadline required so requests can be sorted by when they actually expire.
