About this template
An online donation form should ask for as little as possible before someone gives. This one opens with the amount, suggested at $25 but open to any figure, then asks where the gift should go and who it is from. The card payment happens on the same page, so there is no second site to lose people on.
The dedication field lets people give in memory or in honour of someone, which is one of the most common reasons people donate at all. The anonymity question is asked in plain words, so a donor who does not want to be thanked by name in public can say so.
Replace the designation options with your own programmes, or delete that question if every gift goes to the general fund. The updates box gives you permission to write to donors about what their money did, which is the email most likely to bring a second gift.
