About this template
This event ticket order form sells tickets for a single event without a ticketing platform taking a cut of every sale beyond the card fee. The buyer gives the name for the ticket, pays by card, and you get a list of attendees with their dietary and access needs already filled in.
Dietary and access questions are on the order form because the day before the event is too late to ask them. Caterers need numbers early, and a venue needs to know about step free access before it plans the room.
The phone number is optional and used only if something changes on the day. The last question asks how people heard about the event, which tells you where to spend your promotion budget next time.
Each order is one ticket at a fixed price. If you sell more than one ticket type, such as early bird and standard, publish a copy of the form for each and change the price and label.
Templates are a starting point, not legal or professional advice. Where a form carries consent, medical, or contractual wording, have somebody qualified review it against the rules that apply to you before you publish.

Frequently asked questions
- Can I sell event tickets with a form instead of a ticketing site?
- Yes, for most small and medium events. This form takes card payment through Stripe and gives you the attendee list in your responses, which you can export to a spreadsheet for the door. What you do not get is seat maps or scanned barcodes, so a large venue with reserved seating is better served by a dedicated ticketing tool.
- How do I limit the number of tickets sold?
- Keep an eye on the response count and close the form from your dashboard when you reach capacity. Once it is closed, anyone opening the link sees that registrations have ended instead of a payment page.
- Can one person buy tickets for a group?
- This template sells one ticket per order, which keeps a name on every ticket. For group bookings, add a number field for the group size and use the payment field price for a group ticket, or ask each guest to order their own.
