About this template
A general inquiry form is the catch all on a website for questions that do not fit a sales or support page. This one asks what the inquiry is about, so press, careers and order questions are sorted before anyone reads them, and it asks how and when the person would like a reply.
Those last two questions stop the back and forth of missed calls. If someone says they prefer email, you reply by email. If they want a call in the afternoon, you call in the afternoon and reach them the first time.
The phone number is optional, because some people will leave a form the moment it looks like it will lead to a sales call. Change the topic list to match the departments that answer your inquiries.
Templates are a starting point, not legal or professional advice. Where a form carries consent, medical, or contractual wording, have somebody qualified review it against the rules that apply to you before you publish.

Frequently asked questions
- What is a general inquiry form?
- It is a contact form for questions that are not clearly sales or support. It collects the person's details, the topic, their message, and how they would like to be contacted, so the inquiry reaches the right person and they get a reply in the way they asked for.
- How do I add an inquiry form to my website?
- Publish the form, then use the embed code from the share menu to place it inline on your contact page or as a popup button. You can also link to it from anywhere, including an email signature. Visitors do not need an account to send an inquiry.
- Can inquiries go to different people based on the topic?
- Yes. Connect Slack or email notifications and filter by the topic answer, or send responses to Google Sheets and sort them there. Keep the topic list short so each option maps to one person or team.
