Membership admin goes wrong at renewal, not at signup, and it goes wrong because the original record was thin. This form takes the tier, the start date, the billing frequency, expected usage, health notes, an emergency contact, and acceptance of the rules.
Billing frequency and the cancellation acknowledgement belong on the same page as the signature. That is the pairing you refer back to when somebody disputes a payment, and it costs nothing to collect while they are enthusiastic.
