It runs over three short steps rather than one long page, because the honest answers to how many hours an animal would be alone, and whether the landlord has actually been asked, are the ones people abandon a wall of fields rather than give.
The second step is about the home as it really is, not as it sounds. Owning or renting, whether permission exists yet or is merely assumed, how many people share the space, which age brackets the children fall into, and what the outdoor space is genuinely like. None of these disqualify anyone on their own, and all of them change which animal is the right match.
The third step is the part rescues wish they had in writing. Current animals with their vaccination and neutering status, what happened to previous ones, a vet practice that can be phoned, and three explicit commitments: the length of the animal's life, a home visit before anything is agreed, and returning the animal here rather than passing it on. Those three lines prevent more heartbreak than any interview.
Adapt the housing and experience options to the species you rehome, since a cat application and a horse application share very little beyond the contact details. Rescues that charge an adoption fee usually add it to the confirmation email rather than the form, so that money is never the reason a good applicant stops halfway.
