Prize draws run from a spreadsheet of replies are hard to audit, and the two questions that decide whether a draw is even lawful are usually the ones nobody asked. Age and country are required here, and the terms are accepted explicitly, so the entry list is defensible before the draw is made.
Entries as a fixed set of options rather than a free number keeps the data clean and matches however you are selling tickets. Asking for a preferred prize is optional and does no work in the draw itself, but it tells you which prize to lead with next time.
Keep the marketing checkbox separate and unticked, because bundling consent into the entry itself is what gets a draw complained about. Add a payment field if entries are paid, and close the form on a schedule so late entries never reach the list.
