About this template
Not everyone who spends money for an organisation is an employee. Volunteers buy supplies, members pay for a venue, students pay for a trip. This free reimbursement form gives them one place to ask for it back.
It asks what was bought and why, the date, the amount, who approved the spend, and a receipt. People choose how to be paid back, and a donate it back option lets supporters turn the cost into a gift.
The form deliberately does not collect bank details. The treasurer follows up for those, which keeps account numbers out of a shared inbox.
