About this template
Bulk shirt orders go wrong in the same place every time: the sizes. Somebody sends a message saying they want thirty shirts, and then a week of back and forth follows about how many of each, whether the ladies fit counts separately, and what goes on the back. This form asks for a quantity against every size at once, so the order arrives complete and the total is arithmetic rather than a conversation.
The printing section carries the two things that hold up a job at the press. Placement is a checkbox list rather than a free text field, because front and left chest are different prices and a sentence describing them is not something you can quote from. The personalisation field takes one name and number per line, which is the format most suppliers want anyway and saves retyping a paragraph into a spreadsheet.
Add a payment field if you are collecting money at the same time, and set the garment list to the blanks you actually stock rather than the ones the template ships with.
