About this template
A wholesale order is not a retail order with bigger numbers. It carries a purchase order reference the buyer needs on the invoice, a reseller registration that decides whether tax applies, and payment terms that were agreed before anything shipped. Leave any of the three off the form and somebody in accounts has to go and find it, usually after the goods have already gone out.
The first question the form asks is whether the buyer already has an account, because the answer changes everything that follows. An existing stockist reordering wants the fastest possible path. A new one needs checking before you extend terms to them. Putting that question first means you can route the two cases to different people rather than treating every order as new.
The items field is deliberately free text with a worked example in the placeholder, because trade orders reference product codes that vary by supplier and a fixed dropdown ages badly. Set the payment terms list to the ones you genuinely offer, and remove any you do not want a new account to be able to select.
