About this template
Session feedback is usually a paper sheet left on chairs and swept up by the cleaners. This one is a link or a QR code on the last slide, shown one question at a time so attendees can finish it on the way out of the room.
Knowledge and clarity are separate scales because a speaker can know everything and still lose the room. The question about whether the talk delivered what its description promised is the one programme committees find most useful when choosing next year's line up.
Both open questions are optional, so a rushed attendee can still leave a rating. Make one copy per session, or keep the session question and use a single form for the whole event.
Templates are a starting point, not legal or professional advice. Where a form carries consent, medical, or contractual wording, have somebody qualified review it against the rules that apply to you before you publish.
