About this template
Leave requested in a message thread gets approved and then forgotten. This form records the type of leave, the first and last day away, the number of working days used, who is covering, and whether the person can be reached in an emergency.
The working days field matters because it is the number that goes against an allowance. Asking the person to state it removes the arithmetic argument that otherwise happens at the end of the year, when nobody can remember which Friday was a half day.
Replace the department list with your own, and add a balance field if your team tracks remaining days on the form rather than in a payroll system.
