About this template
A new starter's first morning goes on paperwork or it goes on work. This form moves it earlier: legal name and preferred name, home address, emergency contact, tax reference, equipment needed, and the policy acknowledgements you have to hold.
Note what the payroll section deliberately does not ask for. Bank account numbers do not belong in a form submission you will email around, so the field takes the account name and leaves the numbers to payroll over a channel built for them.
Set your own department list and equipment options, and swap the acknowledgements for the policies in your handbook. Teams hiring across borders usually split the tax section per country.
