About this template
An intake form is the first real conversation with a new client, and most of it is questions you already know you will have to ask. This one asks them once: who they are, what they want built, the budget they have in mind, the date it has to be live, and who signs it off. The answers land before the first call instead of taking up half of it.
The two fields that change the most are the budget range and the approver. A budget band asked as a select rather than an open box gets answered far more often than a blank field, and knowing whether the person filling the form decides or only recommends tells you how long the deal will really take. The upload field catches the brief, the brand guide, and the spreadsheet that would otherwise arrive in three separate emails.
Swap the service list for what you actually sell and the budget bands for your own price ladder. Agencies and studios that quote from a scope usually add a page break before the budget section, so the brief is captured even when somebody stops reading at that question.
