About this template
The first ten minutes of a coaching session are usually spent working out what the session is for. This form spends them beforehand. The client books the hour, says what has happened since you last spoke, and names what would make the time worth paying for, so the session opens on the work.
Asking whether this is a first session or a returning one changes what you prepare, and asking it as a question rather than inferring it from an email address means you get the right answer when somebody books from a new address. The focus list gives people who cannot articulate the problem yet somewhere honest to start.
The default availability runs early mornings through evenings and includes Saturdays, because that is when people who have jobs can be coached. Trim it to yours. Connect Google Calendar and your own commitments block the slots automatically, and every booking is written onto your calendar as it comes in.
