About this template
A first session spent asking questions is a session the client paid for and did not train in. This form gathers the goal, the history, the injuries, and the availability in advance, so the first programme is written before the client walks in.
The injuries field has help text asking about anything that still bothers you occasionally, which surfaces far more than asking about current injuries does. Most people do not consider a shoulder that aches twice a month worth mentioning, and it is exactly what changes a programme.
Asking about sleep alongside training frequency is what stops a plan that looks good on paper and fails in week two. Add a payment field if you sell blocks up front, and keep the goal detail field required, since a vague goal produces a vague programme.
Templates are a starting point, not legal or professional advice. Where a form carries consent, medical, or contractual wording, have somebody qualified review it against the rules that apply to you before you publish.

Frequently asked questions
- What should a personal training intake form ask?
- The main goal and what success looks like, any deadline, training history, injuries and medical background, and the schedule they can actually keep. The schedule decides the programme as much as the goal does.
- Why ask what success looks like?
- Because a goal stated as a number and a date can be planned against, and one stated as getting fitter cannot. Asking in the client's words is also what makes a review conversation possible later.
- Can I take payment for a block of sessions?
- Yes. Add a payment field for a block, or a subscription field for a monthly arrangement, both charged through Stripe into your own account.
