About this template
IT requests stall when the request says what somebody wants but not why, or who will pay for it. This form asks for the category, the exact item, the reason, the date it is needed, and the email of the manager who approves the cost, so the request can be actioned the day it arrives.
Broken equipment is a category of its own, which separates a replacement that has to happen from an upgrade that can wait. The upload field takes a supplier quote or a screenshot of the software page, which saves IT looking it up.
Use this for planned requests and the IT service request form for things that are broken now. Rename the categories to match how your IT team budgets.
