About this template
A ticket that says the computer is broken needs three emails before anyone can start. This form asks for the affected system, how many people are affected, a priority written in plain terms, a one line summary, and what has already been tried, which together is enough to triage without a reply.
The priority options describe impact rather than feelings: work has stopped, a workaround exists, normal, or whenever you can. The appointment field at the end lets the user pick a call back slot, so the engineer does not ring a person who is in a meeting.
Adjust the system list to your own estate, and change the call back hours to match your support desk. Forms submitted here can be sent to a ticketing tool through Zapier.
