About this template
An IT survey is most useful just before budgets are set. This one measures the help desk on a five point scale, asks how quickly tickets are resolved in practice, and rates laptops, the network, and everyday software separately, so you know which part of the service is letting people down.
The ranking question asks staff to order five common complaints, which turns a list of grumbles into a priority list. It is shown one question at a time so it can be finished between meetings.
The department question comes last so it does not feel like the survey is tracking anyone. Change the complaints in the ranking to the issues your tickets show most, and run it again six months later to see if anything moved.
