About this template
A small business without an HR department still has to collect the same paperwork from every new starter. This form gathers it over three short pages: personal details, then pay and tax, then emergency contact and policies, so it can be finished on a phone the week before day one.
The tax withholding form and proof of right to work are required uploads, which are the two documents payroll cannot run without. Bank details are collected here too, so send the link to the new starter directly rather than posting it anywhere public.
The final page asks for a uniform size and a signature against the handbook, health and safety, and data protection policies. Rename those to match your own documents, and link to them in the help text so new hires can read before they sign.
