Restaurant Job Application Form Template for Front and Back of House
A restaurant job application form template lets servers, cooks and bar staff apply with the details a manager actually hires on: position, hours, shifts they can work, experience and food hygiene training. Deoochform makes it free to customize, so add your own roles and share the link.

About this template
Restaurant hiring moves fast and most applicants are on a phone. This form keeps the questions short and puts the deciding ones first: which position, what hours, and which shifts the person can actually cover, so you can see straight away who can work Friday night.
Food hygiene training is asked as a three way choice, because plenty of good applicants are happy to train and front of house roles may not need it. The age question explains itself, since serving alcohol has a minimum age in most places, and a CV upload is optional so experienced staff without one are not turned away.
Edit the position list to match your menu and team. A chain with several sites can add a location dropdown, and a busy kitchen might add a trial shift date question at the end.
Templates are a starting point, not legal or professional advice. Where a form carries consent, medical, or contractual wording, have somebody qualified review it against the rules that apply to you before you publish.

Frequently asked questions
- What should a restaurant job application form ask?
- Contact details, the position applied for, full or part time hours, which shifts the applicant can work, a start date, restaurant experience, food hygiene training, age where alcohol is served, and an optional CV.
- Can I use one form for kitchen and front of house roles?
- Yes. The position dropdown covers both, and you can filter responses by it. If the roles need very different questions, add a rule that shows kitchen questions only to kitchen applicants.
