About this template
A suggestion box on the wall collects two things: nothing, and complaints about the car park. The reason is almost never apathy. It is that people do not believe anything happens to what they put in it, and they cannot tell whether putting their name on it is safe.
This form answers both worries directly. The anonymity question is explicit and offers three honest positions, including the middle one most people actually hold, which is willing to be named but not willing to be pulled into a meeting about it. The name and email fields sit below it and say plainly what they are for, so nobody has to guess whether filling them in invites a conversation.
The impact scale and the ranking question turn a pile of suggestions into a queue. One tells you how much difference the person thinks their own idea would make, the other asks them to order the recurring problems everyone already knows about, and after twenty responses the ranking is the closest thing to an evidence based priority list an internal team is likely to get.
Replace the ranking options with the five things your own people keep raising, since generic ones produce generic data. The most important part is outside the form: publish what happened to the suggestions, because the second round of responses is a direct function of whether anyone saw an outcome from the first.

- How do you make an anonymous suggestion box form?
- Leave the name and email fields optional and say clearly what they are used for, as this template does with its three way anonymity question. Also check that your form is not capturing anything identifying elsewhere, since a truly anonymous form has to be anonymous in the response table as well as on the page.
