About this template
A safety induction is only worth as much as the evidence that people understood it. This quiz gives you that evidence: a dated, named, scored record for each person, sitting next to their confirmation that they attended and know where the assembly point is.
The questions cover fire, reporting, manual handling, protective equipment and spills, which are the five things nearly every induction covers regardless of the site. The one that catches people out is the reporting question, because near misses and blocked fire exits are exactly what goes unreported until something happens.
Site rules differ, and where yours differ from these questions, yours win. Change the options before you send it, because a quiz that marks the correct site answer as wrong will teach people to distrust the induction.
Add a page break before the questions if you want the briefing itself on the form, so somebody can read and answer in one sitting.

- It is evidence, not a substitute. A scored quiz with a name, a date and an attestation shows that a specific person answered specific questions on a specific day, which is considerably more than an attendance sheet. Whether it satisfies your regulator depends on the rules you work under, so check what records you are required to keep.
- How do I stop people simply guessing?
- The multiple answer questions are marked all or nothing, so ticking every box scores zero rather than most of the marks, which removes the strategy people reach for first.
