About this template
A 5K fills up from a link you post once, so the form has to do every job on its own. This one takes the runner details, an age category worked out from the date of birth, a start wave, a shirt size, an emergency contact, the signed waiver, and the entry fee, in a single pass with nothing left to chase afterwards.
The start wave question is the one that keeps the start line safe. Asking for an honest finish estimate as a band rather than a time in minutes gets a usable answer from people who have never raced, and it sorts your field without anybody having to guess. The waiver sits above a real signature field, so what you hold is a dated record tied to that entry rather than a checkbox nobody can prove.
Set the entry fee to your own price, or remove the payment field for a free parkrun style event. Races with a charity place usually add a fundraising target field under the payment block, and a capped event can switch on a submission limit so entries close the moment the last place goes.
Templates are a starting point, not legal or professional advice. Where a form carries consent, medical, or contractual wording, have somebody qualified review it against the rules that apply to you before you publish.

Frequently asked questions
- What should a 5K run registration form ask for?
- Name, date of birth, contact details, an expected finish time, a shirt size, an emergency contact, and a signed waiver. The date of birth matters twice over: it sets the age category for results and it tells you which entrants need a parent to sign.
- Can the form take the entry fee?
- Yes. The entry fee is a Stripe payment field, so a runner registers and pays in one step and you get a paid list instead of a list plus a spreadsheet of who has settled up. Connect a Stripe account and the money goes straight to it.
- How do I collect a race waiver online?
- Keep the waiver statements as separate required checkboxes and put a signature field under them. Each entry then stores which statements were agreed to, a drawn signature, and a timestamp, which is what an insurer asks for when something goes wrong on the course.
- Can I cap the number of entries?
- Yes. Set a submission quota on the form and registration closes by itself once the field is full, so you never sell more places than the course or the permit allows. Runners who arrive after that see a closed message rather than an entry they cannot use.
