About this template
Rental bookings fail at the desk rather than online, almost always over licence eligibility. Asking for the licence number, the issuing country, and how long it has been held means an ineligible booking is caught at submission rather than after the customer has travelled to collect a car.
Collection and return are asked as separate date and time pairs because rental pricing is driven by the hour, and a single free text field produces bookings nobody can price. The date of birth field carries help text about the young driver surcharge so the charge is never a surprise.
The endorsements question is a plain yes or no rather than a form of its own, which is enough to route the booking for manual review without turning the form into an insurance application. Add a payment field for the deposit, and a rule so a different return location reveals a second address field.
