A child travel consent letter is the document a border official asks for when a child arrives without both parents. Grandparents taking a grandchild abroad, one parent travelling after a separation, a school trip or a sports tour all run into it. What officials look for is consistent: who the child is, who is permitting the trip, who is accompanying them, where they are going, and a contact number that works while the trip is happening.
The medical authorisation is the part people leave out and then need. Giving the accompanying adult explicit permission to consent to emergency treatment is what a hospital will ask about, and it is far easier to have on the form than to arrange by phone across time zones.
Two limits worth knowing before you rely on this. Some countries want the letter notarised, and some want it signed by both parents rather than one. Neither is something a form can do for you, so check the requirements for your destination and, where relevant, your airline.
Set the wording to match what your destination expects, and keep a printed copy with the passports rather than only a submission in an inbox.
