About this template
Most trip planning starts with an email chain that takes a week to find out the budget. This questionnaire asks the things a travel agent needs before a first call, one question per screen, over a road trip photo that sets the mood before the client has typed a word.
The ranking question does the heavy lifting. A client who puts food first and nightlife last wants a very different week from one who ranks them the other way round, and the pace scale tells you whether to fill every day or leave afternoons free.
Budget is asked per person with flights included, which removes the most common misunderstanding. Change the currency in the budget options to your own, and add a passport or nationality question if you book international trips.
Templates are a starting point, not legal or professional advice. Where a form carries consent, medical, or contractual wording, have somebody qualified review it against the rules that apply to you before you publish.

Frequently asked questions
- What should a travel planning questionnaire ask?
- Where and when the client wants to go, how long they can be away, who is travelling, a budget per person, what matters most to them, where they like to stay, and anything to plan around such as diets or mobility. That is enough to put together a first proposal.
