About this template
A tour booking form needs to answer three questions before you can confirm anything: which tour, which date, and how many people. This one asks those first, then collects the lead traveller's details and a deposit, so a booking in your inbox is one you can confirm straight away.
The mobile number is required because guides need it on the day, when a group is late or the meeting point has changed. The pickup address is optional, since walking tours often start at a fixed spot.
Dietary needs and mobility requirements share one question, since both change how you plan the day. A guide who knows about a wheelchair user or a coeliac guest the night before can adjust the route or the lunch stop.
Replace the tour names with your own, and set the deposit to what you charge to hold a place. Remove the payment field if you take full payment later.
Templates are a starting point, not legal or professional advice. Where a form carries consent, medical, or contractual wording, have somebody qualified review it against the rules that apply to you before you publish.

Frequently asked questions
- What information should a tour booking form collect?
- The tour, the date, the number of travellers, and the lead traveller's name, email and mobile number, plus any pickup address and dietary or mobility needs. A deposit on the form means the booking is committed before you hold the place.
- Can I take a deposit when someone books a tour?
- Yes. This template charges a booking deposit by card through Stripe when the form is submitted. Set the amount in the builder. Take the balance separately, for example with a copy of the Online Payment Form template.
- Can travellers pick from real available dates?
- This template uses a date field, so travellers suggest a date and you confirm it. If your tours run at set times, swap it for an appointment field, which only shows the slots you have marked as available.
