About this template
A trip needs approving and booking, and those are usually two separate messages to two separate people. This form does both at once: purpose, destination, dates, what to book, the estimated cost, the cost centre, and whether it is billable.
The visa and passport expiry questions are here because they are the ones nobody remembers until the week before. A passport expiring inside six months is a problem you want to find at request time, not at a check in desk.
Replace the department and cabin options with your own travel policy, and add a currency field if your teams book across regions.

- Why ask for the passport name?
- Because a ticket booked in a shortened or preferred name can be refused at check in. Asking for the name exactly as printed is the cheapest way to avoid a rebooking fee.
