About this template
Most consent forms in circulation are a Word document somebody prints, signs, photographs, and emails back. This one keeps the same structure and drops the printer. It names the person consenting, states exactly what they are agreeing to, breaks the terms into separate statements they have to confirm one by one, and closes with a drawn signature and the date.
Separate checkboxes rather than one blanket agreement is the whole point. A single tick proves somebody clicked. Four ticks against four statements show which specific things were explained, which is the difference between a record that stands up and one that does not. The section for under 18s stays hidden behind a yes or no question, so adults are never asked for a guardian name they do not have.
Rewrite the four statements to match your own activity and the law you work under, because the wording is where consent actually lives. Clinics, studios, and researchers usually add a paragraph of the full terms above the checkboxes so the text being agreed to is on the page rather than in an attachment.
