About this template
When a parent books a room for a student, or a company pays for a visiting speaker, the front desk needs proof that the cardholder agreed. This free credit card authorization form names the cardholder and the guest separately, with the reservation number and the dates of the stay.
The cardholder ticks exactly which charges they cover: room and tax, parking, food and drink, incidentals, and damage. A copy of their photo ID can be attached, and the card is identified by type and last four digits only.
Edit the charge list to match your property. Most hotels keep the signed form with the reservation until the guest has checked out and the final bill is settled.
