About this template
Good guest satisfaction survey questions follow the stay in order, so this template is split into three parts: before arrival, during the stay, and looking back. Booking gets its own ease score because a clumsy booking process loses guests before they ever see the room.
The ranking question is the one most surveys leave out. Guests drag price, location, comfort, staff, and breakfast into order, which tells you which of your ratings to worry about. A low breakfast score matters less if your guests ranked breakfast last.
The recommendation score uses a zero to ten scale so it can be read as a Net Promoter Score. Keep the staff mention question, since it gives managers names to thank, and rename the rated areas to match your property.
Templates are a starting point, not legal or professional advice. Where a form carries consent, medical, or contractual wording, have somebody qualified review it against the rules that apply to you before you publish.

Frequently asked questions
- How many guest satisfaction survey questions should I ask?
- Around a dozen at most. This template has thirteen, grouped under three headings so it feels shorter, and the only open boxes are optional. Every question you add past that point costs you responses from the guests you most want to hear from.
- Why include a ranking question?
- Ratings tell you how each area did, but not which areas matter. Asking guests to put five things in order shows where a low score will actually cost you bookings, which is how you decide what to fix first.
