About this template
A questionnaire left on the desk gets filled in by the guests who are furious or delighted and nobody in between. This one is shown one question at a time, which works on a phone from a QR code on the key card, and the first question asks why the guest came so you can read business and holiday stays separately.
The ratings follow the stay in order: check in, the room, how clean it was, and how well they slept. The facilities question is a checklist rather than a set of ratings, so you learn what is actually used before deciding where to spend.
The last question, whether they would book again, is required because it is the one number to track month to month. Swap the facilities list for your own, and add a room number field if you want to follow up on specific rooms.
